Hospice Programs

"We as a family were so glad our mom was placed with such loving and caring people. She felt very good and loved by the people in Hospice who cared for her. We would gladly refer Hospice to other people."
"We really appreciated the staff moving my father into the Lounge for a few hours so we could celebrate his birthday in private with other family members."

Home Hospice Care

For patients who want to live out their lives in the comfort of their own home (or care facility, such as a nursing home).  A registered nurse, social worker, chaplain, certified nurse aide and volunteer provide intermittent visits and on-call services, as agreed with the patient.

Inpatient Hospice Care at Collier Hospice Center

Set amid pleasant landscaped gardens, Collier Hospice Center (CHC) is located on the Exempla Lutheran Medical Center campus.  The 30,000 square foot center is Colorado's first facility designed and built specifically for end-of-life care and is licensed as an acute hospice care facility (it is not licensed as a long-term care facility).

Patients are admitted to CHC when they require 24-hour care interventions to alleviate acute uncontrolled pain and symptoms of their end-of-life disease that are causing distress and discomfort.  Care interventions can include medications, care needs assessment, safety education, and emotional support.  CHC staff members also assist with arranging for the patient's continued care in another location.  Once patients no longer require 24-hour skilled intervention, they may leave CHC and return to their previous living situation or to an alternative living site.

Our beautiful facility features:

·         All rooms, including 24 private patient rooms, are located on one floor

·         Numerous and spacious public and private family areas with abundant natural light and comfortable furnishings

·         Easy access to the outdoors including secure, quiet inner courtyards that are wheelchair- and bed-accessible

·         Low windows that offer full views of the outdoors for bed-bound patients

·         Dining and kitchen area for families

·         Therapeutic tub room for patients

·         Inter-faith chapel

·         Community reference library with internet access

·         Meeting rooms for support groups, grief-work activities, and education to the community and healthcare professionals

·         Offices for our Home Care and Administrative staffs

Hospice care is also provided in all Exempla Healthcare hospitals:  Exempla Lutheran Medical Center, Exempla Good Samaritan Medical Center, and Exempla St. Joseph Hospital.

Progressive Disease

When a patient wants to stay in active curative treatment, but is not quite ready for hospice. The patient is admitted under home care service, and an ELH nurse is called upon to consult with the home care agency on helping with symptom management, such as pain management and other comfort care.

"We really appreciated the staff moving my father into the lounge for a few hours so we could celebrate his birthday in private with other family members."

"I thank God for all your staff. They comforted the whole family throughout this very difficult time in our lives. They also made my father very comfortable. He really enjoyed their patience and friendliness. Thank you very, very much!! May God bless you and all your staff."
