Cardiac Rehab

The Exempla Saint Joseph Hospital Cardiac Rehabilitation Program emphasizes a personalized approach to your recovery through the key components of EXERCISE, EDUCATION and ENCOURAGEMENT.

Every cardiac patient at Exempla Saint Joseph Hospital experiences the healing and education process starting in the hospital and then maintained through an ongoing outpatient program. All members our Cardiac Rehab team of registered nurses, physical therapists, exercise physiologists, dieticians and counselors are dedicated to making your process of recovery and healthy lifestyle changes a positive and rewarding experience for you and your family.

According to published evidence, there are numerous, substantial Patient with Therapistbenefits of Cardiac Rehabilitation, including:

  • Improved symptoms (such as shortness of breath)
  • Improved blood lipid levels (cholesterol readings)
  • Improved general well-being and self-esteem

Exempla Saint Joseph Hospital provides a complete Cardiac Rehabilitation Program which we define in three phases.

Phase I: Inpatient -- While you are in the hospital
Phase II: Outpatient -- After you leave the hospital
Phase III: Outpatient Maintenance - For ongoing lifetime health management

Phase I: Inpatient

As a cardiac patient hospitalized in Exempla Saint Joseph Hospital, you can expect to be provided with education tailored to meet your individual needs to help you start and maintain a healthy lifestyle. Education is provided to cardiac patients with diagnoses including:

  • Heart attack
  • Heart failure 
  • Cardiac intervention (stent or balloon angioplasty)
  • Open heart surgery (coronary artery bypass grafting; valve replacement or repair surgery)

Monitored Ambulation

Monitored ambulation is a walking program designed to increase and regain inpatient strength and endurance. Open heart surgery patients receive monitored ambulation in the hospital a short time after their surgery.

Education

Each cardiac patient receives education and materials on:

  • Self-care and recovery
  • How to live a healthier lifestyle by decreasing your cardiac risk factors
  • Nutrition
  • Activity
  • Cardiac resources and other follow-up materials

Phase II: Outpatient

Our outpatient program offers heart patients the opportunity to participate in their own recovery shortly after leaving the hospital. Outpatient Program Components

  • Exercise - We provide you with an individualized exercise program designed to improve the efficiency and stamina of your heart as well as improve your overall physical health.
  • Education - We supply a wide range of information to both you and your family to help you develop a healthy and positive lifestyle. For a current monthly schedule, please call 303.318.1332 
    • Education Classes are held at convenient times before and after your monitored exercise sessions 
    • Classes are free for patients and their family members

      • Stress Management 
      • Cardiac Medications 
      • Exercise and your Heart 
      • Diabetes and Heart Disease 
      • Cardiac Anatomy & Disease 
  • Encouragement - Positive support from cardiac rehabilitation professionals and fellow participants helps to reduce your anxiety and renews your self-confidence.

Program Features

The Exempla Saint Joseph Hospital Cardiac Rehabilitation Program is comprised of a variety of supportive features such as:

  • Closely monitored exercise classes held at convenient times throughout the day. 
  • Individual consultations and group seminars that focus on managing heart disease, diet changes, exercise and risk factors (e.g., cholesterol, stress, smoking, diabetes, high blood pressure).
  • Active family involvement through support groups and educational classes.
  • Progress reports sent to your physician/s.

Phase III: Outpatient Maintenance Program

Post-Graduate Program

This important phase focuses on lifetime management of your overall health and well-being and features physical fitness and risk factor supervision provided in a minimally supervised setting. This on-going program is available to cardiac rehabilitation graduates to support your continued heart health. When you participate in Phase III, you are choosing to incorporate exercise and risk factor modification into your everyday healthy lifestyle.
