New for 2009: Home When Ready: Nurturing your HEALING Going home when READY, Download PDF here.
Being a good health care provider is about more than practicing good medicine. It's about a warm smile, a kind word, taking the time to really listen, and giving our patients the personalized care they deserve.
During your hospital stay, we want to make sure you receive the best possible care so that you are ready to go home feeling informed and well enough to start your healing process. We know that going home after a hospital stay can be overwhelming. That's why we want to help alleviate any concerns you may have before you go home.
It's all about YOU - preparing for your discharge:
- Talk with your hospital-based physician about your hospital stay and discharge goals when you are admitted.
- During your hospital stay, talk with your hospital-based physician or nurse about what you can expect after your hospital stay, what to do when you get home, and when you will go home.
- Once you have a firm discharge date and time, you will need to arrange for day-time transportation as you will be discharged as soon as you are ready. It is our goal to avoid unnecessary waiting once you are ready to go home.
We strive to get you home safely and efficiently so you can recover more easily from the comfort of your own home. It's important you know that you will not be discharged until your hospital-based physician decides you are ready to return home. If you have any concerns or questions about your hospital stay, please talk with your hospital-based physician or nurse.
We wish you good health and a speedy recovery.
