Knowing Our Care - Understanding Our Results
Our Performance Excellence Vision
We want our hospitals to be among the safest in the nation. Exempla has adopted a model of Four Pillars of Performance which serve to integrate strategy and outcomes throughout Exempla. Through the Four Pillars of Performance, Exempla commits to patient care that is safe, timely, effective, efficient, equitable and patient-centered.
Four Pillars of Performance
Pillar I - Safest Hospitals - promote a culture of respect and accountability that provides the highest degree of safety to our patients, families and caregivers.
Pillar II - Best Patient Experience - consistently provide care that is patient-centered, compassionate and respectful of patient and family needs.
Pillar III - Best Teams - Build team capacity through effective communications, skill development and informed decision-making to meet the needs of our patients, families and the communities we serve.
Pillar IV - Financial Stewardship - Become recognized as a value-added care provider.
Our monthly "report card," is a document that is shared across the organization which details all of the goals and current outcomes for each of the pillars of performance. We believe it is essential to regularly grade ourselves and the progress of each of our hospitals on a month-by-month basis.
These measures, which are widely recognized and used by hospitals nationwide, fall under four main categories:
Click on any of the categories above to learn more about the specific measures we monitor each month. In each section you will find:
- The specific measures we use for each category - from preventing central line infections to guarding against pressure ulcers.
- What each measure means
- Why each is important to your care
- What steps we take to ensure your care and safety for each measure
- A current report on how we are doing at achieving our goals for the safety measure.
Our ultimate goal is to be among the very best hospitals - performing in the top 10% of all other hospitals in the country -- at providing your care and ensuring your safety on each one of these measures.
To get there, we annually set goals for each measure at each of our hospitals that will move us toward that ultimate goal. And then we measure our performance each month against the goal for the year at each hospital.
You will notice that the annual goals for our hospitals may differ from each other. That is because they have each started from a different place and need to "stretch" at different rates to each our ultimate goal of best-in-the-nation performance. But that is their common aim: to provide you the highest standard of care every time you are in our care.
How You Can Use This Information
You can use the information found here in three ways:
- To learn more about the steps our hospitals take to provide you the safest care with the best outcomes
- To compare the performance of our hospitals with other hospitals in Colorado and the nation (see How We Compare) as you decide which hospital to use for your own care
- To be an active partner with us in your own care.
We think that last way of using this information may be the most important. For each of the measures you will find a list of the specific steps we take to ensure the highest level of safety and best outcomes for you. You or a family member or friend can use the steps as your own check list. When you are in our care and you see a doctor or one of our staff do something that is not on one of the check lists, ask them why. And if you don't see them doing something on the check list, ask them why not. Our doctors and staff will welcome hearing from you. We all share the same hope and aim of making you better. The best outcomes and the safest care happen when doctors, staff, patients and family work together for that common goal.
